RESTORATIVE BREAST SURGERY AFTER MASTECTOMY
The goal of breast surgery is to provide a similar breast in shape, size, position, and texture to the opposite breast. Women are now referred to a plastic surgeon following their diagnosis of cancer to allow the choice of immediate or delayed reconstruction. Most women who have experienced mastectomy consider reconstruction.
Breast reconstruction can be performed in one or two stages, but generally require at least two procedures.
The first operation involves the creation of the breast mound; subsequent operations involve revisions to achieve symmetry with the opposite breast, as well as the creation of the nipple-areola complex. Several types of operations can be performed, using a newly shaped breast with the use of a breast implant, the patient’s tissue flap, or a combination of the two. A tissue flap is a section of skin, fat, and muscle that is moved from the abdomen, back, or another area of the body to the chest area. Immediate breast reconstructive surgery in NYC will typically provide the most aesthetically pleasing result and is performed at the time of mastectomy.

BREAST RECONSTRUCTION OPTIONS
The following are the two most common breast reconstruction options:
Implant Reconstruction: A procedure which combines the skin expansion technique with the subsequent insertion of a permanent implant. After the general surgeon removes the breast tissue, the plastic surgeon places a breast implant to form the contour of the breast.
In selected patients, it will be possible to proceed to one-stage breast reconstruction using a permanent implant.
Reconstruction of the Breast with Flaps: A procedure involving the creation of the breast mound using the patient’s tissue. The tissue remains healthy because it maintains its blood supply. Breast reconstruction with autologous tissue can be performed with pedicled or with free flaps. The two most common types of pedicled flap surgeries are the TRAM flap (transverse rectus abdominis muscle flap), which uses tissue from the abdomen, and the latissimus dorsi flap, which uses tissue from the upper back.
Free flaps can be harvested from the abdomen or the back of the thighs, and use microsurgical techniques for the anastomoses.
